英文的道歉信跟信來的格式差不自多，基本上就是：
Mr. /Mrs./Ms. XXX(如果是跟你有比較親近的關系可以用Dear，不過那麼親近的人也不用寫道歉信了吧= =)
I am sorry for.......because ..... (說上2、3句就行，然後可以扯上相關的其它內容)
XXX（最後簽上自己的名字）
